Job summary

Responsibilities

Assesses health status of patients and families by history and exam
Initiates and orders procedures, consultations, laboratory tests; prescribes medications
Educates patients on health promotion, disease prevention, and management of illnesses
Documents findings and care plan in the medical record
Consults with physician per established standards and protocols
Provides age-appropriate care with knowledge of pediatric/adult needs
Collaborates with physicians to develop and adjust plans of care
Participates in team meetings to support interdisciplinary care delivery
Maintains patient confidentiality and adherence to regulations
Develops and implements plan of care based on diagnoses and patient needs
