Responsibilities

• Develops and reviews collaborative practice agreement with physician.
• Assesses health status via history and exam.
• Initiates therapies and prescribes medications per protocol.
• Provides health promotion and disease prevention teaching.
• Documents care in the medical record and coordinates referrals.
• Participates in team meetings to support interdisciplinary care.
• Reviews patient records with collaborating physician at least every 3 months.
