Job summary

Blue Shield of CA is seeking a Clinical Service Coordinator, Advanced to support care coordination for our most vulnerable members. The role involves connecting members with resources, coordinating services, and working with the Care Management team under the Manager of Care Management.

Ideal candidates have 5 years of relevant experience, high school diploma or GED, and preferred certifications in patient advocacy, health coaching, or care navigation.

Qualifications

• Requires a high school diploma or GED (or 1 extra year of relevant experience).
• 5 years of prior relevant experience.
• Certification/training as patient advocate/health coach/care navigator desired.
• Health insurance/managed care experience across Commercial, IFP, Medicare, Medi-Cal.
• Experience with community resources and advocacy.
• Strong interpersonal and organizational skills.

Responsibilities

• Coordinate care for vulnerable members to overcome barriers and access services.
• Connect members with needed resources and provide individualized care coordination.
• Report to the Manager of Care Management and contribute to a high-performing team.
• Demonstrate critical thinking and a commitment to continuous learning.
