Job summary

Grace at Home is seeking an APP Lead to oversee training and day-to-day management of NPs, PAs, and RNs, ensuring high-quality patient care in home settings.

The role requires leadership in a clinical, community-based environment, with responsibility for documentation, regulatory compliance, and collaboration with interdisciplinary teams to improve member outcomes. Telehealth visits are included in the scope.

Qualifications

  • Master’s degree in nursing or related field; NP license required.
  • 2+ years leadership/management in clinical settings; Medicare/Medicaid experience required.
  • 2+ years clinical experience with complex chronic conditions; LTSS preferred.
  • Strong verbal and written communication; group facilitation skills.
  • Proficient in MS Office (Word/Excel/PowerPoint/Outlook).

Overview

Reporting to the Regional Chief Medical Officer, the APP Lead is responsible for training and day to day management of Nurse Practitioners (NPs), Physician Assistants (PAs), and RN’s, ensuring effective collaboration, proper clinical management, documentation of visits, and high‑quality patient care delivery to Grace at Home family members. This role involves team leadership, performance monitoring, overseeing an interdisciplinary team to meet organizational goals, improve member outcomes, and provide exceptional patient care. The assistant manager will also manage processes to ensure members, and their families, receive timely and effective navigation support, both telephonically and in the field. This position is accountable for ensuring Grace at Home is positioned to innovatively deliver on its promise. Duties & Responsibilities
  • Mentor clinical team members, providing clinical guidance, regular performance evaluations, constructive feedback, and supporting professional development. Administrative time accounts for 0.4 FTE of the APP Lead’s time
  • Implement evidence‑based practices and clinical protocols.
  • Monitor and evaluate patient care delivery to maintain high standards of quality and safety.
  • Ensure staff schedules optimize productivity, meeting patient care needs, maintaining high standards of care and safety in compliance with regulatory requirements.
  • Address and resolve patient or staff concerns promptly and professionally.
  • Coordinate orientation and continuing education for clinical staff, promoting opportunities for professional growth.
  • Serve as a liaison between clinical staff and administrative leadership.
  • Participate in interdisciplinary team meetings to enhance patient care coordination.
  • Ensure all staff adhere to regulatory standards, including HIPAA and OSHA requirements.
  • Maintain accurate documentation of clinical activities, training, and compliance records.
  • Conduct patient visits in‑person in the patients' homes and/or via telehealth maintaining a patient panel equivalent to 0.6 FTE
  • Address and resolve patient concerns promptly and professionally.
  • Collaborate with business partners such as Home Health and Hospice agencies, assisted living facilities, Primary Care providers, Specialists, hospital staff and other preferred partners.
  • Collaborate with multiple care disciplines such as RN’s, and cross functional team‑members to provide high‑quality care.
  • Will participate in after hours on‑call coverage rotation and rotating evening a weekend patient care coverage.
Qualifications
  • Education/Experience: Master’s degree in nursing or related science field required
    • Certification/Licensure: Active and unrestricted NP license required
  • Clinical: Must have 2+ years of strong leadership and management experience in a clinical setting; community‑based care experience preferred. Experience with Medicare/Medicaid is a must.
  • Must have 2+ years of clinical experience treating complex chronic conditions and high‑needs patients, Medicare and LTSS experience preferred
  • Excellent verbal, written, presentation, and group facilitation skills.
  • Must have strong critical analytic, thinking and communication skills necessary to evaluate and respond to team progress, anticipate needs and other related team dynamics.
  • Computer Knowledge: Excellent computer skills required particularly Microsoft applications including Word, Excel, PowerPoint, and Outlook. Willingness to explore and learn new or unfamiliar applications that may benefit the clinical team
  • Culture: Good judgement, impeccable ethics, and a strong team player; desire to succeed and grow in a fast‑paced organization.

The working environment and physical requirements of the job include:

In‑office work is performed indoors in a traditional office setting with air conditioning, artificial light, and an open workspace.

In this position you will need to communicate with customers, vendors, management, and other co‑workers in person and/or via phone or telehealth. Regular use of the telephone and e‑mail for communication is essential. Sitting for extended periods is common. Must be able to receive ordinary information and to prepare or inspect documents. Lifting of up to 10 lbs. occasionally may be required. Good manual dexterity for the use of common office equipment such as computer terminals, calculator, copiers, and FAX machines. Good reasoning ability is important. Able to understand and utilize management reports, memos, and other documents to conduct business.
