Job summary

UW Health is seeking an Advance Care Planning Program Coordinator in Madison to coordinate activities across Population Health, Ambulatory Care Operations, and legal teams. This full-time hybrid role requires strong collaboration and strategic planning.

You will develop workflows, monitor data, and drive equity-focused improvements in access to quality care, working with Marketing to craft health messaging and patient outreach.

Qualifications

• Master's degree in a relevant field (Social Work, Nursing, Chaplaincy, Business).
• At least 5 years of healthcare experience with advance care planning or care coordination.
• Current professional license as applicable; RN license preferred; CAPSW/CISW/LCSW/APC Chaplain certification preferred.

Responsibilities

• Coordinate program activities with Population Health, Ambulatory Care Operations, Ambulatory Social Work, UW Health Legal, Health Information Management, and others.
• Develop workflows and lead optimization of advance care planning implementation with emphasis on equity and access.
• Monitor and report on data to assess needs and interventions.
• Evaluate and improve program outcomes based on evidence, patient and provider satisfaction.
• Collaborate with Marketing and Communications to create and deliver advance care planning messaging.
