Job summary

Wellstar Health System is seeking an Advance Care Planning Facilitator to engage patients and families in shared decision-making. You will educate providers, patients, and the community on advance care planning and ensure proper documentation in the EMR.

In this role, you will develop presentations, collaborate with care teams, and help implement ACP processes across Wellstar facilities. Strong communication and project management skills are essential, with occasional travel to outpatient sites.

Qualifications

• Masters Social Work or Masters in another field is required.
• Excellent communication skills and ability to interact with all stakeholders in the health system.
• Project management experience strongly preferred.
• Ability to travel to outpatient facilities within WellStar service area.

Responsibilities

• Advance Care Planning Facilitation and conduct discussions with patients and families.
• Educate providers, patients, families and community on advance care planning.
• Document sessions in EMR and maintain schedules.
• Develop and deliver education/outreach sessions and presentations.
• Collaborate with ACP manager to improve policies and tracking of outcomes.
