Job summary

Qualifications

• Master's degree in Social Work or related field.
• GA licensure: LCSW or LMSW required at hire.
• Healthcare setting experience and knowledge of ACP preferred.
• Excellent communication and organizational abilities essential.

Responsibilities

• Facilitate advance care planning discussions with patients and families.
• Educate providers, patients, families, and community on ACP.
• Develop and maintain ACP policies, documentation, and tracking.
• Document ACP sessions in EMR and coordinate scheduling.
• Lead education/outreach initiatives and collaborate with ACP manager.
