San Gabriel/Pomona Regional Center (SG/PRC) seeks a Service Coordinator to provide comprehensive case management to adults and their families. You will collaborate with the Regional Center, healthcare and community resources to ensure high-quality supports and thriving outcomes.

The role requires a Bachelor's degree in a related field, at least one year of relevant experience, and Spanish bilingual ability.

Qualifications

• Bachelor's degree in a related field is required.
• 1 year of experience with individuals with developmental disabilities or related field required.
• Bilingual in Spanish is required.
• Master's degree can substitute for required experience.

Responsabilités

• Coordinate services with Regional Center and other service providers.
• Explain available resources to individuals and families so they understand options.
• Conduct in-person meetings with individuals, families, vendors and agencies to plan and coordinate services.
• Develop and implement individual Program Plans (IPP) tailored to each client.

Formation

Bachelor's degree in social work, psychology, human development, sociology, public health nursing, or related field

Master's degree can substitute for required experience
