About MyPlace Health

myPlace Health was founded in 2021 by mission-aligned healthcare leaders and organizations that are committed to drastically improving health outcomes, quality and experience for vulnerable older adults and frail seniors. We specialize in providing value-based, comprehensive care and coverage for older adults with significant needs so they can thrive in the homes they love and in the communities they cherish.

Our mission is simple: to enable older adults to live the independent lives they deserve. We pursue this mission through our myPlace PACE (Program of All-Inclusive Care for the Elderly) model, which provides seamless primary care, integrated health plan coverage, personalized social engagement, and customized services delivered in the participant’s preferred place.

myPlace Health is building a mission-driven team that shares our passion for redefining the way older adults experience care as they “age in place” in the community. This is a unique opportunity to take on one of our country’s most challenging healthcare problems and join a fast-growing, dynamic team as we prepare to scale our mission to serve more markets.

About This Role

Are you a mission-driven healthcare professional with a passion for helping seniors live their best, most independent lives? If so, we invite you to join our team! In this role, you’ll have the unique opportunity to provide direct medical care to PACE participants as part of a supportive and collaborative Interdisciplinary Team (IDT). Together, you’ll develop and carry out care plans that truly make a difference in the lives of older adults.

As a Primary Care Provider (Nurse Practitioner or Physician Assistant) you’ll meet participants where they are—whether that’s at the center, clinic, their home, virtually, or an external site of care. With a holistic approach to health and wellness, you’ll empower participants to achieve their goals, improve their health outcomes, and thrive in their communities.

This is more than a job; it’s a calling to serve with compassion, build meaningful connections, and be part of a team dedicated to transforming lives.

What does success look like as our Primary Care Provider?

  • Be a cornerstone of participant care by completing assessments, contributing to care planning, and collaborating on service determination requests.
  • Build meaningful relationships as you grow and manage a panel of participants, ensuring their unique needs are met.
  • Deliver exceptional care by evaluating, initiating, and adjusting medications, diagnostic studies, and treatments with thoughtful supervision.
  • Document with purpose, recording participant interactions, assessments, and next steps in our EMR system within established timeframes.
  • Coordinate specialized care by making referrals to specialists and consulting as needed.
  • Educate and empower participants by performing history and physicals, teaching preventative care, and facilitating advanced planning discussions.
  • Respond with agility by evaluating and treating participants wherever they are—whether at the center, in their home, virtually, or in a skilled nursing facility (SNF).
  • Collaborate with the best by working closely with behavioral health, pharmacy, and other team members to ensure holistic participant care.
  • Stay proactive by ordering, interpreting, and following up on lab tests and results, performing selected procedures, and escalating abnormalities when needed.
  • Support continuity of care by documenting chronic conditions via HCC capture and providing on-call coverage as needed.
  • Provide additional support within the scope of your license to ensure participants receive the care they need, when they need it most.

What does the ideal Primary Care Provider look like?

  • Heart for service: A passion for and commitment to serving high-risk seniors and frail older adults.
  • Specialized experience: Experience working with complex and older populations, with at least two years of experience as an RN caring for medically complex individuals prior to becoming an NP, or with at least two years of experience as a PA with complex and older populations.
  • Exceptional communication: Strong interpersonal and communication skills to connect with participants, families, and team members.
  • Team player with leadership skills: Ability to work both independently and collaboratively in a team setting.
  • Educational background: Master’s degree and graduation from an accredited NP/PA program; Certification as a Nurse Practitioner with prescriptive authority; Graduation from an accredited School of Nursing or Physician Assistant program.
  • Licensing and certifications: Active, unrestricted NP or PA license and CPR certification; RN or PA license in the State of California, if applicable.; Valid CA driver’s license with a good driving record, personal transportation, and auto insurance (as required by State law).
  • Bilingual skills preferred: Fluency in English and Spanish is highly desirable to better serve our participants.
  • Clinical expertise: Current knowledge of chronic care and geriatric issues, with a commitment to best practices in these areas.
