Job Description

The Nurse Practitioner or Physician Assistant will oversee the Community Health Worker (CHW) team, evaluate assessments, and approve their completion. This role provides holistic, quality care to members enrolled in managed care plans, focusing on closing care gaps, addressing social health needs, and re-engaging members with primary care to improve quality of life and reduce costs. The position is primarily home-based with supplemented telehealth visits for complex medical populations.   • Salary Range: $150,000 - $200,000 (dependent on experience)
  • Reports To: Senior Vice President & Principal
  • Direct Reports: Community Health Workers, Patient Care Navigator
Location & Travel

San Francisco, CA; Up to 80% travel

Work Type & Schedule

Position Responsibilities
  • Collaborate with individuals and families to address health, mental, and social issues.
  • Partner with physician practice leaders and lead a health team.
  • Manage health of homebound and complex medical patients.
  • Communicate effectively across healthcare settings and with diverse populations.
  • Conduct assessments, develop treatment plans, and prescribe medications as needed.
  • Document patient information accurately and maintain compliance with regulations.
  • Lead and coordinate team efforts to close care gaps and improve health outcomes.
Qualifications
  • California licensure as Nurse Practitioner or Physician Assistant.
  • National Board Certification.
  • Graduate from an accredited Master’s or Doctoral program.
  • 1-3 years healthcare management and patient care experience preferred.
  • Experience with home health care for complex, homebound patients preferred.
  • Excellent communication skills; bilingual abilities a plus.
  • Willingness to travel throughout the assigned area.
Knowledge & Competencies
  • Proficiency with EMR systems, MS Office, and data entry.
  • Understanding of Medicare and Medicaid regulations.
  • Strong judgment, problem-solving, and interpersonal skills.
  • Organizational skills to manage multiple priorities effectively.
