Qualifications

• VA RN? NP license in Virginia required.
• Current national board certification as a Nurse Practitioner.
• BLS certification required.
• Eligible for prescriptive authority and CS registration.
• Clinical experience in internal medicine or primary care preferred.
• Strong clinical assessment, diagnostic, communication, and organizational skills.
• Experience with EMR systems.
• Experience with chronic disease management and preventive care.
• Experience working in a multidisciplinary healthcare environment.

Responsibilities

• Conduct histories and physicals for adult patients.
• Diagnose and develop treatment plans for adults.
• Manage acute and chronic conditions (HTN, diabetes, lipids, asthma, COPD).
• Order and follow up on labs and tests; interpret results.
• Prescribe medications per policies and laws.
• Monitor response to treatment and adjust plans.
• Provide preventive care and wellness counseling.
• Educate patients on diagnoses, medications, and lifestyle changes.
• Perform in-office procedures within scope and privileges.
• Coordinate referrals to specialists and communicate with care team.
• Maintain accurate EMR documentation.
• Participate in care coordination and population health initiatives.
• Collaborate with physicians and staff on complex cases.
• Identify patients requiring urgent or higher levels of care.
• Maintain compliance with regulations and standards.
