Job summary

Total Patient Care, LLC, a well-established Internal and Family Medicine practice in Brick, NJ, is hiring a Nurse Practitioner (APN) – Internal Medicine to provide high-quality primary care to adult patients.

You will work collaboratively with physicians and the clinical team to manage preventive care, chronic disease, and acute conditions, performing wellness visits, ordering tests, and educating patients. NJ license and board certification required.

Qualifications

• Current New Jersey APN license.
• National Board Certification (AANP or ANCC).
• Current DEA and CDS registration.
• BLS Certification.
• Minimum of 1-2 years of experience in Internal Medicine, Primary Care, or Family Medicine preferred.
• New graduates will gain experience and mentorship from our established professionals on staff.
• Strong clinical judgment, communication, and patient education skills.
• Experienced Nurse Practitioners will benefit from our vast patient pool and knowledgeable staff of professionals serving the NJ community for over 30 years.

Responsibilities

• Evaluate, diagnose, and treat acute and chronic medical conditions.
• Perform annual wellness visits, routine physicals, and follow-up appointments.
• Order and interpret laboratory and diagnostic tests.
• Prescribe medications and develop individualized treatment plans.
• Educate patients on disease prevention, medications, and healthy lifestyle choices.
• Managing chronic conditions.
• Coordinate care with specialists and other healthcare providers.
• Maintain accurate documentation in the electronic medical record (EMR).
• Participate in quality initiatives and value-based care programs.
