Job summary

Tristate Health Pay, LLC in Secaucus, NJ is seeking a Nurse Practitioner to deliver comprehensive primary and preventive care to adult patients in a Tristate internal medicine practice, with support in North Bergen, NJ. You will independently manage care within your licensure and collaborate with physicians and the interdisciplinary team to promote strong clinical outcomes and excellent patient experience.

Qualifications

• Master's or Doctoral degree in nursing required.
• Active NP license in each state where services are provided.
• Current national board certification for adult primary care (FNP/ANP/AGPCNP).
• Minimum 2+ years of APN experience in internal medicine/adult primary care or equivalent.

Responsibilities

• Provide comprehensive primary and preventive care to adult patients.
• Collaborate with physicians and interdisciplinary team to promote outcomes and patient experience.
• Manage care within licensure, privileges, and scope of practice with physician support as needed.
• Develop and adjust evidence-based treatment plans using guidelines and diagnostics.
• Coordinate with care team to support chronic disease management and preventive services.
• Ensure accurate and timely documentation in the EHR and maintain patient safety.
