Total Patient Care, LLC is seeking a compassionate Nurse Practitioner (APN) to join our Internal Medicine practice. The ideal candidate will provide high-quality primary care to adult patients while working collaboratively with our physicians and clinical team to promote exceptional patient outcomes.

Responsibilities include evaluating, diagnosing, and treating acute and chronic conditions; performing wellness visits and routine physicals; ordering tests; prescribing medications; educating

Qualifications

Current New Jersey APN license and board certification.
DEA and CDS registrations are required.
BLS certification and strong clinical judgment.
1–2 years in Internal/Family Medicine preferred; new grads welcome.

Responsibilities

Evaluate, diagnose, and treat acute and chronic conditions.
Perform annual wellness visits and routine physicals.
Order and interpret labs and diagnostic tests.
Prescribe medications and develop treatment plans.
Educate patients on prevention, medications, and healthy lifestyles.
Coordinate care with specialists and EMR documentation.
