Job summary

Lifepath Systems in McKinney, TX is seeking a QMHP-CS to provide routine case management, skills training, medication training and crisis interventions within a trauma-informed, person-driven care model.

You will help clients access medical, self-help, social, vocational, financial and educational services, document outcomes, and work both independently and as part of a collaborative team to support community members.

Qualifications

• Bachelor’s degree in psychology, social work, or related field from a four-year college or university.
• At least two years of experience in working with the community, preferred.
• Bilingual (Spanish/English) preferred.

Responsibilities

• Provide routine case management, skills training, and medication training with crisis interventions.
• Support psychosocial rehabilitation services based on trauma-informed, person-driven care.
• Help clients navigate medical, self-help, social, vocational, financial, educational services.
• Ensure timely documentation and compliance with state standards and Lifepath Systems policies.

Maximize resources to enhance Lifepath Systems operations and support client’s strengths and needs, and helping to navigate medical, self-help, social, vocational, financial, educational, and other individualized services. Demonstrates integrity in role and by ensuring state standards and Lifepath Systems policies and procedures are met (i.e., timeliness, documentation, case management). Work independently with integrity and collaborative as a team member and community builder. Continuously improve in measurable ways on how to support individuals in service, community, and LifePath Systems services as a leader in the QMHP-CS role.

Qualifications
