Job summary

LifePath Systems is seeking a QMHP-CS to deliver outpatient case management, skills training, and psychosocial rehabilitation in trauma-informed, person-driven care models. This role links individuals to medical, social, vocational, and educational resources, coordinating plans across community, home, and school settings.

Under supervision, you will document plans, meet targets, and work with community partners to reduce recidivism and improve mental health stability.

Qualifications

• Bachelor’s degree in psychology, social work, or related field.
• At least two years of related mental health experience.
• Knowledge of Trauma-Informed Care practices.
• Experience with court/justice systems is highly desired.
• Must have reliable transportation and a valid driver’s license.

Responsibilities

• Provide outpatient routine case management, skills training, medication training, and psychosocial rehab services.
• Coordinate and document service plans to meet individual needs in the community, home, school, and legal system.
• Collaborate with clinicians, sheriffs, and community partners to increase access to services.
• Travel by car up to 80% to meet caseload needs.
