Job summary

RIPIN is seeking a Community Health Worker to support adults with special needs and their families within the Conflict Free Case Management program. You will provide person-centered, culturally sensitive peer support and assist with care coordination in home and community settings.

The role emphasizes motivational interviewing, data collection, and collaboration with state agencies to connect clients with Long Term Services and Supports while maintaining detailed records and reporting.

Qualifications

• High School diploma or GED is required or equivalent experience.
• CHW certification is preferred; certification must be earned within 18 months of hire.

Responsibilities

• Engage with adults in home and community settings to provide peer support.
• Coordinate care and referrals within the Conflict Free Case Management program.
• Document records timely and accurately for reporting and evaluation.
• Support clients in identifying goals and accessing Long Term Services and Supports.
