Job summary

South Alabama Regional Planning Commission seeks an ADRC Coordinator to conduct comprehensive assessments and determine supports for callers, ensuring follow-up to obtain services. The role also assures accuracy of the resource database and coordinates public education and outreach across the service area.

The ADRC Coordinator handles intake duties, provides information and referrals, and supports short-term case management.

Qualifications

• Bachelor’s degree in Social Work, Behavioral Sciences, Psychology, Geriatric Studies, or related field with four years of work experience in aging/home health programs.
• Must obtain certifications: Inform USA’s Community Resources Specialist; Person Centered Training (PCT); SHIP within one year of hire.
• Must be Alabama Medicaid Certified Case Manager or obtain certification within 90 days.

Responsibilities

• Develops and implements public education and communication strategies to increase awareness of the AAA’s programs.
• Networks with community groups to strengthen relationships and program recognition regionally and at the state level.
• Provides community education sessions as needed.
• Performs intake duties for callers and walk-ins.
• Provides information and referrals, assesses needs, identifies resources, and follows up to ensure needs are met.
• Assists with completion of applications for assistance and short-term case management.
