Job summary

San Simeon by the Sound Center for Nursing and Rehabilitation in New York seeks an experienced Registered Nurse to serve as Assistant Director of Nursing (ADON). The ADON partners with the Director of Nursing to oversee high-quality resident care, clinical operations, staff supervision, and regulatory readiness.

This leadership role focuses on quality assurance, infection prevention, survey readiness, and collaboration with interdisciplinary teams to promote coordinated care.

Qualifications

• Current and unrestricted New York State RN license required.
• Prior nursing leadership or supervisory experience required.
• Skilled nursing facility, long-term care, or post-acute rehabilitation experience strongly preferred.
• Knowledge of CMS and New York State skilled nursing regulations.
• Strong clinical assessment, leadership, communication, and organizational skills.
• Ability to effectively supervise staff and manage multiple clinical priorities.
• Experience with electronic medical records, preferably PointClickCare or a similar system, preferred.

Responsibilities

• Assist the Director of Nursing with the overall management and daily operation of the Nursing Department.
• Provide leadership, direction, and clinical support to RNs, LPNs, CNAs, nursing supervisors, and other nursing personnel.
• Help ensure adequate staffing and appropriate assignment of nursing personnel based on resident acuity and facility needs.
• Monitor the quality and appropriateness of resident care and address identified concerns promptly.
• Assist with implementation and enforcement of nursing policies, procedures, and standards of practice.
• Participate in resident assessments, care planning, clinical meetings, and interdisciplinary team meetings.
• Assist with investigations involving incidents, accidents, allegations, grievances, and other resident-care concerns.
• Support facility compliance with New York State Department of Health, CMS, and other applicable regulatory requirements.
• Participate in survey preparation, survey management, plans of correction, and ongoing survey readiness.
• Participate in Quality Assurance and Performance Improvement (QAPI) initiatives and identify opportunities to improve clinical outcomes.
• Monitor documentation and ensure nursing records are accurate, timely, and compliant with applicable requirements.
• Assist with staff education, competency evaluations, orientation, coaching, counseling, and performance management.
• Monitor infection-control practices and reinforce appropriate clinical and safety protocols.
• Assist with reducing avoidable hospitalizations, falls, pressure injuries, medication errors, and other adverse clinical outcomes.
• Serve as a nursing leadership resource during emergencies and significant changes in resident condition.
• Collaborate with Administration, Rehabilitation, Social Services, Dietary, Medical Providers, and other departments to promote coordinated resident care.
• Assume additional nursing leadership responsibilities in the absence of the Director of Nursing when assigned.
