Job summary

Capitol Hill Skilled Nursing & Therapy is seeking an ADON Case Manager to lead nursing staff and coordinate resident care. The role focuses on RAI oversight, care planning, and ensuring quality outcomes for residents in our skilled nursing facility.

The position requires an Oklahoma RN or LPN license, current CPR/First Aid, and at least 1 year in long-term care. You will collaborate with interdisciplinary teams, manage MDS processes, and participate in discharge planning and staff development.

Qualifications

• Current Oklahoma RN or LPN license.
• First Aid and CPR certification up to date.
• 1+ year of experience in a Skilled Nursing Facility or Long Term Care preferred.

Responsibilities

• Conduct daily interdisciplinary meetings to track therapy minutes, RUG changes, patient status changes.
• Perform pre-admission screenings to determine level of care and eligibility for skilled nursing services reimbursable by Medicare.
• Plan, schedule and collect MDS assessments data according to Medicare schedule.
• Coordinate and assist with discharge planning.
• Conduct daily rounds to evaluate resident care and assist nursing staff.
• Assist with supervision and orientation of nursing personnel.
• Ensure personnel are assigned responsibilities per education and ability.
• Assist with supervision of CNAs and one-on-one teaching as needed.
• Be on call for staffing needs.
