Sierra Health and Wellness seeks a Care Coordinator - SUD Counselor in Roseville, CA to support adolescents in our outpatient program. You will assess clients, develop treatment plans, and coordinate referrals and discharge planning in a therapeutic setting.

The role requires a high school diploma or GED, California counselor certification, CPR/First Aid, and a valid driver’s license. We offer comprehensive benefits and a supportive team.

Qualifications

• Must possess a High School diploma or GED.
• Certification as a Drug and Alcohol Counselor by a California Regulatory Agency.
• Current CPR and First Aid certification.
• Valid Driver’s License and insurable under Sierra Health and Wellness Centers automobile insurance policy.

Responsabilités

• Assess and monitor client behaviors in treatment for chemical and alcohol abuse.
• Develop treatment plans to reduce or eliminate substance use and related issues.
• Make referrals for mental health or medical needs; coordinate SSI processes if needed.
• Document client behavior in group participation and counseling; ensure notes are complete.
• Provide crisis management and counseling; lead group therapies for clients and families.
• Coordinate discharge planning and community outreach; respond to emergencies as needed.
