Sierra Health and Wellness is seeking a Care Coordinator - SUD Counselor to serve adolescents in Roseville, CA within an outpatient PHP program. The role involves assessing needs, planning treatment, and coordinating referrals while supporting families in a therapeutic setting.

Qualifications

• Must possess a High School diploma or a GED.
• Certification as a Drug and Alcohol Counselor by a California Regulatory Agency.
• Minimum one year of experience in drug and alcohol counseling.
• HIPAA familiarity and client confidentiality.
• Valid license/certification in drug and alcohol counseling as required by CA licensing boards.
• Current CPR and First Aid certification.
• Valid driver’s license and insurability under Sierra Health and Wellness Centers policy.

Responsibilities

• Assess and monitor clients in treatment for chemical and alcohol abuse.
• Develop and implement treatment plans to reduce substance use and related issues.
• Make referrals for mental health or medical needs; assist with SSI processes when appropriate.
• Document client behavior in group and individual sessions; provide crisis management.
• Lead group therapies and family education in a therapeutic setting.
• Coordinate treatment and discharge planning; support emotional needs of clients.
