Job summary

Honeycomb Wellness Center in Novato, CA seeks a full-time Case Manager/Counselor for our Intensive Outpatient SUD program. The role provides client support, conducts assessments, develops treatment plans, and evaluates progress to enhance care quality and patient satisfaction.

The SUD Counselor will integrate individualized case management, perform discharge planning, and coordinate with the Program Director. Weekend coverage and on-call duties may be required to meet client needs.

Qualifications

• Certified Substance Abuse Counselor (CADC or equivalent).
• 2+ years of experience in SUD counseling.
• CPR and First Aid certification is required.
• Must pass a background check and a TB test.
• Must have a valid California driver's license.

Responsibilities

• Provide ongoing client support and mentoring.
• Conduct comprehensive assessments and develop treatment plans.
• Implement and evaluate individual client needs and outcomes.
• Promote continuity of care with case management, weekly reviews, and discharge planning.
• Serve as central receiver of clinical data and coordinate with the Program Director.
• Be flexible with on-call crisis response and weekend scheduling.

Job description

We are looking to hire a full time Case Manager/Counselor at our Intensive Outpatient SUD facility in Novato!

The SUD Counselor is responsible for the support and mentoring of clients. SUD Counselorswill provide ongoing support and expertise through comprehensive assessments, treatmentplanning, implementation and overall evaluation of individual client needs while enhancing thequality of patient management and satisfaction. SUD Counselors are to always promote thecontinuity of care through integrating individualized case management, weekly reviews anddischarge planning. Act as the central receiver of pertinent clinical data and works closely withthe Program Director in the formulation of treatment planning, immediately responding to a crisisand planning for the needs of clients to ensure efficacy of service delivery. SUD Counselor must be flexible and able to come in while on call for an emergency and be open to schedulechanges when required (example: working on weekends). The SUD Counselor can have up to12 clients on their caseload at a time.

Requirements:

• Certified Substance Abuse Counselor (CADC or equivalent)
