Job summary

Youth Opportunity is seeking a Case Manager for the West Central Ohio Academy in Troy, OH. This full-time role supports assessment, treatment planning, family involvement, and discharge preparation under licensed supervision.

The position involves coordinating services, documenting progress, and engaging with youth and families through treatment teams and community partners to ensure effective aftercare and transitions.

Qualifications

• Bachelor’s degree in counseling, social work, psychology, rehabilitation, special education, or a related human services field
• Two (2) years of experience working with children with serious emotional disturbances or substance use problems required; this experience is preferred rather than required for applicants with a master’s degree
• Strong communication, documentation, and organizational skills
• Knowledge of group leadership and group dynamics
• Ability to coordinate services and work effectively with youth, families, and multidisciplinary teams

Responsibilities

• Provide case management and coordinate communication among youth, families, treatment staff, and referring agencies
• Complete assigned screenings and assessments under licensed mental health supervision
• Prepare performance summaries, treatment reviews, and required case documentation
• Coordinate transition, aftercare, and discharge planning
• Maintain post-release information and tracking data
• Serve as a liaison with families, referring agencies, and community partners
• Help prepare youth and families for visits, conferences, and aftercare services
• Identify community resources and coordinate referrals
• Participate in treatment-team meetings and service planning
• Support compliance reviews and quality improvement activities
• Report incidents and concerns related to youth safety or inappropriate conduct
