Job summary

Responsibilities

• Conduct group and/or family sessions and document as required.
• Co-facilitate sessions and modify behaviors.
• May perform initial and follow-up assessments.
• Prepare individual treatment plans for patients.
• Ensure documentation is complete and timely.
• Liaise with referral sources and patients.
• Provide crisis intervention when needed.
• Plan for aftercare and liaise with insurers.
• May perform case management and refer to other programs.
• Evaluate patient needs and referrals.
