Job summary

Perimeter Behavioral Health of Forrest City is seeking a Case Manager to support adolescents in a residential behavioral health program. The role collaborates with a treatment team to coordinate services, document care, and assist with discharge planning.

The position requires a bachelor’s degree in a human or social services field (preferred) or an associate degree with relevant experience; candidates must be 21+.

Qualifications

• Bachelor’s degree in a human or social services field preferred.
• Associate degree with at least two years of experience working with children or adolescents may qualify.
• Additional work experience may substitute for a degree in the required field.
• Knowledge of clinical treatment and social casework in mental health or substance use settings is important.
• Must be at least 21 years old.

Responsibilities

• Participate in treatment planning and treatment team meetings.
• Coordinate with therapists and physicians on case documentation and discharge planning.
• Communicate with referral sources and other mental health professionals about patient progress with consent.
• Plan and evaluate therapeutic passes with therapists and families.
• Collaborate with the team to address patients’ social, legal, family, and education needs.
• Complete accurate case records and documentation in a timely manner.
• Communicate treatment information clearly to patients, families, and caregivers.
• Respect patients' cultural and religious beliefs.
• Follow policies on patient rights, confidentiality, and safety.
• Report suspected abuse or neglect as required by policy and law.
• Participate in quality management activities.
