Job summary

Mindoula Health’s Collaborative Care Program (CCP) is expanding to Oregon. The Adolescent Behavioral Health Integration Clinician– Associate Level collaborates with PCPs and psychiatric consultants to provide short-term, evidence-based care to adolescents enrolled in the program.

Under supervision, you will screen for mental health and substance use, deliver brief interventions, monitor medications, track response, and refer to specialty care as needed.

Qualifications

• Master’s degree in Social Work, Counseling, Psychology, or related field.
• Clinical Social Work Associate (CSWA) / Professional Counselor Associate (PCA) / LMFTA / QMHP eligibility.
• Experience working with adolescent populations.
• Experience with EMRs + telehealth workflows.
• Proficiency with technology and using multiple applications.
• Located in Oregon (majority of services are remote).

Responsibilities

• Screen for mental health and substance use disorders.
• Provide short-term, evidence-based interventions.
• Support medication adherence and monitor side effects.
• Track clinical response and adjust plan as needed.
• Refer for specialty care when appropriate.
• Participate in weekly psychiatric case consultation.
• Document in EHR following model workflows.
• Re-engage patients who fall out of care.
• Practice under supervision of independently licensed clinicians.
