NEWCare, Inc. in Wisconsin is seeking a full-time Admissions Coordinator/Social Services Designee to work with residents, families, and staff.

You will manage census development, community relations, and education with referral sources while helping with admissions processing and social service planning. This role focuses on identifying residents’ social, emotional, and psychological needs, assisting with discharge planning, and supporting care conferences.

Qualifikationen

• High school diploma or equivalent.
• Associate degree in Social work, healthcare, or related field preferred.
• One year experience in healthcare or social services setting; two to three years preferred.
• Must recognize and understand the psychosocial and emotional needs of residents.
• Great communication skills; both verbal and written.

Aufgaben

• Conduct admission interviews with the resident and family for psychosocial history and assessment, identifies concerns, develops plans/goals, and completes advance directives.
• Acts as liaison between families, residents, and staff for complaints, concerns, requests, and information, ensuring care standards and quality.
• Advocates for and educates residents, family members, and employees about Residents’ Bill of Rights.
• Cultivates/maintains professional relationships in local health care markets.
• Generates referrals by building relationships with hospitals, physicians, and community resources.
• Coordinates tours, admissions, insurance verifications and pre-certs.
• Facilitates admission decisions and ensures positive admission experience for customers and families.
• Assists with annual in-service on resident rights and discharge planning.
