Job summary

Home & Health Care Management, Inc. is seeking a qualified Registered Nurse to perform intake and case management for applicants and participants in their homes or via telehealth in the Sacramento area.

The role involves assessments, care planning for HCBA, CCT and ALW programs, and collaboration with an interdisciplinary HCBA team to ensure seamless enrollment and ongoing care. Travel and in-office presence are required.

Qualifications

• Current California State license as a Registered Nurse.
• Ability to work independently and with others at all levels within the organization.

Responsibilities

• Review medical records and plan of treatment to ensure appropriate care.
• Communicate with providers, patients and housing representatives regarding care needs.
• Collaborate with HCBA case management team and interdisciplinary staff.
• Institute tracking of quality indicators and reports to improve patient outcomes.
• Coordinate with HCBA team members via Teams or in-person meetings.
