Job summary

Chapters Health System is seeking an Admission RN to assess patients in residences or hospitals and initiate care into our services. The role ensures a seamless transition to Care Navigation and acts as an ambassador for the organization.

Requirements include an active RN license in the applicable state, at least 1 year of nursing experience (hospice/hospital preferred), and proficiency with EMR/EHR systems. Bilingual Spanish is a plus, and a valid driver’s license is required.

Qualifications

• Current RN license in the practicing state.
• Minimum 1 year nursing experience; hospice or hospital preferred.
• Experience with EMR/EHR systems.
• Strong clinical assessment skills; ICU/ED background helpful.
• English communication required; Spanish bilingual preferred.

Responsibilities

• Complete clinical admissions for all services.
• Explain services and address patient/family questions empathetically.
• Obtain consents and initial assessments.
• Communicate with physicians and initiate care plans.
• Coordinate referrals and discharge planning.
