Job summary

Chapters Health System is seeking an Admission RN to assess patients in their home or hospital, initiating care and coordinating with Care Navigation. You will serve as a liaison, ensuring seamless transition to services while upholding the organization's standards.

Requirements include active RN license in the state, 1 year nursing experience (hospice or hospital preferred), and experience with EMR/EHR systems. Spanish language skills are a plus; driving mobility is required.

Qualifications

• Current RN license in the state where employed.
• Minimum one year of nursing experience; hospice or hospital preferred.
• Experience with EMR/EHR systems and patient documentation.
• Strong clinical assessment skills and ability to educate patients.
• Able to communicate effectively in English; Spanish a plus.

Responsibilities

• Completes clinical admissions for all services including explaining services to patients and families.
• Identifies admission barriers and provides responses to concerns.
• Educates patients/families on admission requirements and initial assessments.
• Initiates care plans, orders, and communicates with physicians.
• Represents the company at facilities and coordinates referrals across service lines.
• Maintains timely documentation and uses SBAR for clinical communication.
• Works with Care Navigation staff to ensure timely follow-up and smooth transitions.
• Adheres to safety, privacy, and regulatory requirements.
