Job summary

BAYADA Home Health Care is seeking a Full-Time Admissions Registered Nurse (RN) to join our home health visit team in southern Rhode Island. You will complete OASIS SOC assessments for adult and geriatric patients throughout the Providence area and manage urgent home-based clinical needs with minimal on-site support.

Requirements include an active Rhode Island RN license and at least one year of relevant hands-on experience.

Qualifications

• Current Rhode Island nursing license in good standing.
• Graduation from an accredited and approved nursing program.
• At least one year of hands-on medical experience with adult/geriatric patients in the past two years.
• Medicare Certified Home Health experience and OASIS experience preferred.

Responsibilities

• Complete OASIS SOC assessments for adult/geriatric patients in southern Rhode Island.
• Triage and manage urgent clinical situations in the home setting.
• Perform medication reconciliation, teaching, and safety monitoring.
• Conduct comprehensive wound assessment, care and documentation.

BAYADA Home Health Care is currently seeking a Full-Time Admissions Registered Nurse (RN) to join our home health visit team. You will complete OASIS Starts of Care (SOC) for adult and geriatric patients throughout southern Rhode Island. One year prior clinical experience as a licensed RN is required and Home Health and OASIS experience is strongly preferred. You will be an integral member of a multi-disciplinary health care team that provides skilled nursing and rehabilitative care to clients, affording them the opportunity to remain at home. How You’ll Make an Impact

• Use strong head‑to‑toe assessment skills with the ability to identify subtle changes in patient condition.
• Utilize your experience with acute and post‑acute care (e.g., CHF, COPD, wound care, IV therapy, post‑surgical care).
• Triage and manage urgent clinical situations independently in the home setting.
• Perform medication reconciliation, teaching, and safety monitoring.
• Perform comprehensive wound assessment, care, and documentation.

Qualifications for a Registered BAYADA Nurse

• A current nursing license in good standing in the state of Rhode Island.
• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
• At least one year of current or recent hands‑on medical experience within the past two years with the adult/geriatric population in acute care, emergency department, home health, hospice, ICU, med‑surg, sub‑acute care or rehab units (medical or skilled nursing settings).
• Medicare Certified Home Health experience and OASIS experience is strongly preferred.

Compensation & Schedule

Competitive pay ranges with a weekly pay schedule. Pay: Weekly Base Pay, depending on qualifications and subject to negotiations $58.00 - $63.00 per point.
