Job summary

BAYADA Home Health Care is seeking a Full Time Admissions Registered Nurse (RN) to join our home health visit team covering Delaware County and parts of Philadelphia. You will assess patients in their homes, coordinate admissions, and collaborate with a multidisciplinary team to deliver skilled nursing and rehabilitative care.

Successful candidates have a PA license, at least one year of recent relevant experience, and are prepared to work independently in diverse home environments.

Qualifications

Current nursing license in good standing in Pennsylvania.
Graduate of an accredited nursing program with transcript or diploma.
At least one year of hands-on medical experience with adult/geriatric populations.
Previous home health experience required.

Responsibilities

Perform head-to-toe assessments and identify changes in patient condition.
Triage and manage urgent clinical situations in the home setting.
Perform medication reconciliation, teaching, and safety monitoring.
Conduct comprehensive wound assessment, care, and documentation.
