Job summary

St. Francis Hospice in Titusville, FL is seeking an Admissions Registered Nurse to join our hospice care team. The RN will evaluate patient/family needs, determine eligibility, and develop the initial plan of care in collaboration with the Intake Coordinator and Clinical Service team.

Active Florida RN license is required; ADN is required and BSN preferred. Hospice, home health, or palliative care experience is preferred. We offer a comprehensive benefits package and a $6,000 sign-on bonus.

Responsibilities

• Determine eligibility and prepare the initial plan of care for patient/family.

St. Francis Hospice (SFH) is growing, and we need an Admissions Registered Nurse to join our team. SFH is a well-established, mission-driven healthcare organization dedicated to providing exceptional hospice and palliative care services throughout our community.

The Admission Registered Nurse (RN) is responsible for determining the eligibility and preparation of patient/family need and the initial written plan of care. Position works closely with the Intake Coordinator, Clinical Service Department, and the Chief Operating Officer to ensure timely processing of all referrals. Position serves as a resource for questions/issues from the health care community and SFR staff.

Associate Degree in Nursing (ADN) required. Bachelor of Science in Nursing (BSN) preferred. Current and active Florida Registered Nurse (RN) License required. Hospice, home health, palliative care, or related experience preferred.
