Job summary

VITAS Healthcare in Texas is seeking a Hospice Admissions Nurse to bridge hospital or community care with hospice, guiding patients and families through a pivotal transition with clarity, empathy, and confidence.

You will conduct comprehensive clinical assessments to determine eligibility, educate about hospice services and Medicare benefits, and coordinate with physicians and interdisciplinary teams to admit patients.

Qualifications

• RN with a valid state license.
