Job summary

VITAS Healthcare seeks a Hospice Admissions Nurse to bridge hospital or community care with the comfort of hospice. You will guide patients and families through a pivotal transition with clarity, empathy, and confidence.

As a nurse in this role, you will perform comprehensive assessments, educate about Medicare benefits, collaborate with medical teams, and manage admission documentation, all while upholding patient-centered care and dignity.

Qualifications

• Registered Nurse with a valid state license.
• Minimum 2 years of nursing experience (hospice, palliative, ER, ICU, home health or case management preferred).
• Strong clinical assessment and critical thinking skills.
