Job summary

Ascend Hospice is seeking an Admissions Registered Nurse (RN) to evaluate patients and families on admission, outline referrals, and initiate individualized care plans. This role requires RN licensure in the operating state and 2–4 years in hospice settings, with Bachelor's degree preferred.

You will collaborate with the clinical team to implement the hospice philosophy, coordinate the plan of care, and engage local hospitals to support admissions.

Qualifications

• 2-4 years of hospice experience.
• Licensed as a Registered Nurse in the state of operation.
• Associate's degree is required; Bachelor's degree preferred.

Responsibilities
