Job summary

LifeSource Hospice is seeking an experienced RN Hospice Case Manager to join our interdisciplinary team serving patients in Bergen County, NJ. You will manage a caseload of hospice patients, coordinate care across disciplines, and ensure compliance with CHAP standards and regulations.

You will perform nursing assessments, provide skilled visits, and collaborate with physicians, social workers, and other IDT members while traveling to patient homes and facilities within the county.

Qualifications

• Active NJ RN license in good standing.
• Minimum 1 year experience in hospice, home health, or long-term care.
• Familiarity with CHAP hospice standards preferred.
• Experience with WellSky EMR and BetterRx is a plus.

Responsibilities

• Manage a hospice caseload of approximately 15 patients.
• Conduct comprehensive nursing assessments and develop individualized plans of care.
• Provide skilled nursing visits and symptom management.
• Coordinate care with physicians, social workers, chaplains, aides, and other IDT members.
• Ensure documentation meets CHAP standards and regulatory requirements.
• Utilize WellSky EMR for timely and accurate charting.
• Perform medication reconciliation and coordinate e-prescribing via BetterRx.
• Provide patient and family education related to disease progression and end-of-life care.
• Participate in IDT meetings and care planning.
• Collaborate with facilities and community partners.
• Travel to patient homes and facilities within Bergen County.

This role is responsible for managing a caseload of hospice patients, coordinating care across disciplines, and ensuring compliance with hospice regulations and standards.

The ideal candidate has hospice, home health, or long-term care experience and is comfortable working independently in the field.

Key Responsibilities

• Manage a hospice caseload of approximately 15 patients

• Coordinate care with physicians, social workers, chaplains, aides, and other IDT members

Schedule

• Full-time

• Monday-Friday

• On-call rotation (one weekend per month in Bergen county vicinity)
