Job summary

Moments Hospice is seeking an Admissions RN for a full-time, on-the-road role. You will assess patients for hospice eligibility, coordinate admissions, and build referral networks to support timely, compassionate care. A valid RN license and a driver's license with vehicle are required.

The role emphasizes autonomy, relationships with care teams, and delivering high-quality hospice services. Expect a Monday–Friday schedule with a specialized admissions focus and benefits including a company car

Qualifications

• Requires a valid RN license in the state of practice.
• 1-2 years nursing experience preferred.
• Driver's license, auto insurance, and a vehicle are required.
• Compassion and integrity are essential for hospice care.
• Previous hospital/physician practice/LTC/ALF experience is a plus.

Responsibilities

• Participate in information gathering, analysis, education and form completion of admissions.
• Evaluate patients for hospice appropriateness via medical records review as requested.
• Obtain agreement for admission of referred patients and families meeting admission criteria.
• Analyze data collected to facilitate/obtain admission orders.
• Build effective referral source relationships to support Moments Hospice's high quality and responsiveness.
• Maintain clinical and professional competence.
• Develop relationships with admissions and patient care staff.
• Educate patients and families about hospice and palliative care.

Nurses looking for autonomy and variety should consider practicing in Hospice Care. As an Admissions RN, you will join a team that is dedicated to making our patients' final days, weeks, and months as comfortable as possible.

Hours for this position will be Monday-Friday 12:00 pm-8:00 pm

Responsibilities:

• Participation in the information gathering, analysis, education and form completion aspects of the admissions process
• Evaluate patients for hospice appropriateness, e.g. medical records review as requested.
• Obtain agreement for admission of referred patients and families who meet admission criteria.
• Analyze data collected to facilitate/obtain admission orders
• Build effective referral source relationships to support the high quality and responsiveness expectations associated with Moments Hospice
• Maintain clinical and professional competence.
• Take the initiative in developing effective relationships internally with the appropriate admissions and patient care resource persons.
• Utilize interpersonal skills to help appropriately educate and address any questions the patient and/or patient’s family has concerning hospice and palliative care

Requirements:

• Valid Registered Nurse (RN) license
• Driver's license, insurance and vehicle
• 1-2 years of nursing experience (preferred)
• Utmost compassion and integrity
• Currently licensed as a R.N. in the state where duties are performed.
• Previous hospital, physician practice, LTC or ALF experience
• Strong Presentation Skills
• Previous sales or service experience preferred
