Job summary

Memorial Hermann Health System seeks a Clinical Nurse Liaison to perform clinical assessments of patient referrals and present findings to physicians for admission.

The role supports hospital marketing of services, participates in quality improvement, and builds strong relationships with external referral sources, social workers, discharge planners and families to increase utilization of Memorial Hermann programs.

Qualifications

• Graduate from an accredited school of professional nursing.
• Current Texas RN license required; BLS certification required.
• Three years nursing experience in rehabilitation, ER, SICU/NICU or related field.
• Experience in case management or similar roles preferred.

Responsibilities

• Conduct clinical assessments of patient referrals and present findings to physicians for admission.
• Participate in process improvements related to admissions and hospital programs.
• Market hospital services to external referral sources and healthcare professionals.
• Serve as a resource to directors to identify barriers to referrals and improve access.
