Lakeland Hills Center seeks an Admissions Coordinator to streamline the admissions process, support census goals, and coordinate with physicians, social workers, and discharge planners. You will manage patient intake, verify eligibility, and supervise daily admissions activities to ensure a smooth resident experience.

Ideal candidates have a high school diploma or GED and prefer a healthcare-related bachelor's degree or LVN/RN license, with 2 years of admissions experience.

Qualifikationen

• Must possess, as a minimum, a high school diploma or GED.
• A Bachelor's Degree in health care or related field Preferred.
• Two years' Admissions experience preferred.
• LVN or RN license preferred.

Aufgaben

• Manage the admissions process to ensure a positive experience for residents, families and staff.
• Plan, direct and monitor daily Admissions Department operations to meet census goals.
• Admit patients to the health care facility and complete admission forms.
• Screen patients against admission criteria and verify information.
• Coordinate with Nursing/Case Management to ensure appropriate admissions.
• Develop relationships with physicians, social workers and discharge planners to maximize referrals.
• Maintain bed availability records and ensure timely patient intake and paperwork.
