Job summary

Pine Ridge Post Acute in Washington state is seeking an Admissions Nurse who is compassionate, organized, and clinically confident to guide new residents through the admissions process. Qualifications

• Current Washington State RN license in good standing.
• Minimum of 2 years nursing experience.
• Graduate of an accredited nursing program.
• Current CPR/BLS certification.
• Experience with electronic medical records; PointClickCare preferred.

Responsibilities

• Review referrals and medical information to determine admission appropriateness.
• Complete comprehensive clinical reviews including diagnoses, medications, therapy needs, labs, infection status, behavioral concerns, and functional abilities.
• Communicate with hospitals, discharge planners, physicians, families, pharmacies, transportation providers, and other referral partners.
• Collaborate with Nursing, Administration, Therapy, Social Services, Dietary, and other members of the interdisciplinary team.
• Review physician orders, medication lists, therapy recommendations, and discharge documentation before admission.
• Coordinate with nursing and ancillary departments to ensure rooms and resources are ready for new residents.
• Complete or oversee nursing admission assessments and required documentation.
• Ensure accurate medication reconciliation, physician orders, consents, care plans, and EMR documentation.
• Initiate admission care plans and communicate resident needs to the care team.
• Coordinate infection prevention and isolation precautions when needed.
• Maintain accurate admission logs, referral tracking, and required reports.
• Serve as a clinical resource for referral sources regarding Pine Ridge services.
• Support quality improvement efforts related to admissions, transitions of care, and reducing rehospitalizations.
• Help ensure admission practices remain compliant with federal regulations, Washington State requirements, and facility policies.
• Protect resident privacy and HIPAA compliance.
