Job summary

Pine Ridge Post Acute is seeking an Admissions Nurse to support the resident transition from hospital to skilled nursing. You will review referrals, perform clinical assessments, and coordinate with hospitals, families, and the care team to ensure smooth admissions.

The role requires a Washington license, 2+ years of nursing experience, CPR/BLS, and strong organizational skills. This position is part-time and on-site, with a focus on compliant, high-quality transitions of care.

Qualifications

  • Current Washington State license in good standing.
  • Minimum 2 years of nursing experience.
  • Graduate of an accredited nursing program.
  • Current CPR/BLS certification.
  • Strong clinical assessment, critical-thinking, and decision-making skills.
  • Excellent communication and organizational skills.
  • Knowledge of Medicare, Medicaid, and post-acute care regulations is preferred.

Responsibilities

  • Review clinical referrals and determine admission appropriateness.
  • Complete comprehensive clinical reviews, including diagnoses, medications, therapies, labs, infection status, and functional abilities.
  • Communicate with hospitals, discharge planners, physicians, families, pharmacies, and other referral partners.
  • Collaborate with Nursing, Administration, Therapy, Social Services, Dietary, and other team members.
  • Review physician orders, medication lists, therapy recommendations, and discharge documentation before admission.
  • Coordinate with departments to ensure rooms and resources are ready for new residents.
  • Complete or oversee nursing admission assessments and required documentation.
  • Ensure accurate medication reconciliation, orders, consents, care plans, and EMR documentation.
  • Initiate admission care plans and communicate resident needs to the care team.
  • Coordinate infection prevention and isolation precautions when needed.
  • Maintain admission logs, referral tracking, and required reports.
  • Serve as a clinical resource for referral sources regarding Pine Ridge services.
  • Support quality improvement efforts related to admissions and transitions of care.

Part-Time

$30-$53 per hour

At Pine Ridge Post Acute, the admissions process is more than paperwork - it's the beginning of a resident's next chapter. We're looking for an Admissions Nurse who is compassionate, organized, clinically confident, and enjoys working with people to help make transitions into skilled nursing as smooth and comfortable as possible.

What You'll Do
  • Review clinical referrals and medical information to determine admission appropriateness.
  • Complete comprehensive clinical reviews, including diagnoses, medications, treatments, therapy needs, labs, infection status, behavioral concerns, and functional abilities.
  • Communicate with hospitals, discharge planners, physicians, families, pharmacies, transportation providers, and other referral partners.
  • Collaborate closely with Nursing, Administration, Therapy, Social Services, Dietary, and other members of the interdisciplinary team.
  • Review physician orders, medication lists, therapy recommendations, and discharge documentation before admission.
  • Coordinate with nursing and ancillary departments to help ensure rooms and resources are ready for new residents.
  • Complete or oversee nursing admission assessments and required documentation.
  • Ensure accurate medication reconciliation, physician orders, consents, care plans, and electronic medical record documentation.
  • Initiate admission care plans and communicate resident needs to the care team.
  • Coordinate infection prevention and isolation precautions when needed.
  • Maintain accurate admission logs, referral tracking, and required reports.
  • Serve as a clinical resource for referral sources regarding services available at Pine Ridge.
  • Support quality improvement efforts related to admissions, transitions of care, and reducing rehospitalizations.
  • Help ensure admission practices remain compliant with federal regulations, Washington State requirements, and facility policies.
  • Protect resident privacy and maintain HIPAA compliance.
What We're Looking For
  • Current Washington State license in good standing.
  • Minimum 2 years of nursing experience .
  • Graduate of an accredited nursing program.
  • Current CPR/BLS certification .
  • Strong clinical assessment, critical-thinking, and decision-making skills.
  • Excellent communication and organizational skills.
  • Ability to manage multiple referrals and priorities in a fast-paced environment.
  • Basic computer skills and experience with electronic medical records; PointClickCare experience is preferred .
  • Knowledge of Medicare, Medicaid, managed care, and post-acute care regulations is preferred.
  • Experience in skilled nursing, rehabilitation, hospital case management, admissions, utilization review, or discharge planning is a plus.
