Job summary

Graduate Post Acute is seeking an Admissions Nurse for a full-time role. You will perform comprehensive initial assessments, document histories, and coordinate with physicians to plan admissions and placements.

The ideal candidate holds an RN license with BLS, an associate’s or BSN degree, and experience in emergency, critical care, or admissions nursing.

Qualifications

• Active RN license with state requirements.
• Current BLS certification (CPR).
• Associate's or Bachelor's degree in Nursing; BSN preferred.
• Experience in emergency, critical care, or admissions nursing is desirable.

Responsibilities

• Conduct comprehensive initial assessments of incoming patients to determine illness severity and care needs.
• Collect and document detailed medical histories, including current medications, allergies, and previous treatments.
• Collaborate with physicians and interdisciplinary teams to develop preliminary care and placement plans.
• Serve as the first point of clinical contact, providing compassionate, patient-centered care during admission.
• Communicate clearly with patients and families about the intake process, care expectations, and next steps.
• Monitor for signs of medical distress during intake and elevate care as needed.
• Maintain up-to-date knowledge of hospital admission policies, infection control protocols, and triage guidelines.
