Job summary

Graduate Post Acute is seeking an Admissions Nurse to join our team in a full-time role with daily pay benefits. The position focuses on comprehensive admissions assessments, effective communication, and smooth transitions of care for each patient and family we serve.

As an Admissions Nurse, you will coordinate with physicians and interdisciplinary teams, document findings in electronic health records, and ensure timely placement plans while upholding our core values of compassion and clinical

Qualifications

  • Active RN license (state-specific requirements apply).
  • Valid BLS Certification (CPR).
  • Associate’s or Bachelor’s degree in Nursing (BSN preferred).
  • Experience in emergency, critical care, or admissions nursing is highly desirable.
  • Strong clinical assessment and communication skills.
  • Proficient with electronic health record systems and clinical documentation.

Responsibilities

  • Conduct comprehensive initial assessments of incoming patients to determine illness severity and care needs.
  • Collect and document detailed medical histories, including current medications, allergies, and previous treatments.
  • Collaborate with physicians and interdisciplinary teams to develop preliminary care and placement plans.
  • Serve as the first point of clinical contact, providing compassionate, patient-centered care during admission.
  • Communicate clearly with patients and families about the intake process, care expectations, and next steps.
  • Monitor for signs of medical distress during intake and elevate care as needed.
  • Maintain up-to-date knowledge of hospital admission policies, infection control protocols, and triage guidelines.
