Graduate Post Acute in Philadelphia is seeking a Registered Nurse (RN) to provide skilled clinical care, monitor residents, and implement individualized care plans across day and night shifts. The role requires an active PA RN license, BLS, and the ability to care for multiple residents in a fast-paced setting.

Starting pay ranges from $37 to $46 per hour with daily pay options, and benefits include tuition reimbursement, health and dental plans, 401(k) with match, and shift differentials.

Qualifications

Responsibilities

• Provide resident care per physician's plan and written care plan.
• Follow and implement care plans developed by the Nurse Manager.
• Perform medication administration and treatments per plan of care.
• Monitor residents' conditions and promptly report changes to the clinical team.
• Maintain accurate documentation per physician orders and facility policies.
• Collaborate with interdisciplinary teams for coordinated resident care.
• Ensure infection control protocols and nursing regulations are followed.

Responsibilities for Registered Nurse (RN):

• Provide resident care in accordance with acceptable standards of nursing practice and the physician’s plan of treatment, as well as the written plan of care.

• Follow and implement care plans developed by the Nurse Manager.

• Perform medication administration and treatments in accordance with the plan of care (POC).

• Monitor and assess residents’ conditions, promptly reporting changes to appropriate clinical team members.

• Maintain accurate, timely, and complete documentation in accordance with physician orders, facility policies, and regulatory standards.

• Collaborate with interdisciplinary teams to ensure coordinated and high-quality resident care.

• Ensure compliance with infection control protocols, safety standards, and nursing regulations.

Qualifications for Registered Nurse (RN):

• A current, unencumbered active license to practice as an RN in the state of Pennsylvania.

• Valid BLS Certification (CPR) required.

• Ability to manage care for multiple residents simultaneously in a fast-paced healthcare environment.

• Strong clinical assessment, communication, and documentation skills.
