Job summary

Graduate Post Acute in Philadelphia is seeking an Admissions Nurse to join our full-time team. The role focuses on comprehensive assessments, clear communication, and a smooth transition of care for patients and families.

The ideal candidate holds an active RN license, BLS, and a BSN or higher; experience in emergency or admissions nursing is preferred. Starting pay is $45 per hour with daily pay options, and benefits include health and dental, 401(k), and tuition reimbursement.

Qualifications

• Active RN license; state-specific requirements apply.
• Valid BLS Certification (CPR).
• Associate’s or Bachelor’s degree in Nursing; BSN preferred.
• Experience in emergency, critical care, or admissions nursing is desirable.
• Strong clinical assessment and communication skills.
• Proficient with electronic health record systems (EHR).

Responsibilities

• Conduct comprehensive initial assessments of incoming patients to determine illness severity and care needs.
• Collect and document detailed medical histories, including current medications, allergies, and previous treatments.
• Collaborate with physicians and interdisciplinary teams to develop preliminary care and placement plans.
• Serve as the first point of clinical contact, providing compassionate, patient-centered care during admission.
• Communicate clearly with patients and families about the intake process, care expectations, and next steps.
• Monitor for signs of medical distress during intake and elevate care as needed.
• Maintain up-to-date knowledge of hospital admission policies, infection control protocols, and triage guidelines.
