Job summary

Graduate Post Acute is seeking an Admissions Nurse for a full-time role in Philadelphia. The position runs Monday through Friday, 2pm-10pm, with daily pay and starting pay at $45 per hour (all inclusive).

The Admissions Nurse will conduct comprehensive assessments, document histories, collaborate with care teams, and ensure a smooth transition of care for patients and families.

Qualifications

• RN license active (state-specific).
• Valid BLS CPR certification.
• Associate or BSN in Nursing; BSN preferred.
• Experience in emergency, critical care, or admissions helpful.
• Strong assessment and communication skills.
• Proficient with electronic health records and documentation.

Responsibilities

• Perform comprehensive initial patient assessments on admission.
• Document medical histories, meds, allergies, and prior care.
• Collaborate with physicians to build preliminary care plans.
• Serve as the first clinical contact during admission.
• Explain intake steps and next steps to patients/families.
• Monitor for distress and escalate care as needed.
• Stay current with admission policies and infection control.

Responsibilities for Admissions Nurse:

• Conduct comprehensive initial assessments of incoming patients to determine illness severity and care needs.
• Collect and document detailed medical histories, including current medications, allergies, and previous treatments.
• Collaborate with physicians and interdisciplinary teams to develop preliminary care and placement plans.
• Serve as the first point of clinical contact, providing compassionate, patient-centered care during admission.
• Communicate clearly with patients and families about the intake process, care expectations, and next steps.
• Monitor for signs of medical distress during intake and elevate care as needed.
• Maintain up-to-date knowledge of hospital admission policies, infection control protocols, and triage guidelines.

Qualifications for Admissions Nurse:

• Active RN license (state-specific requirements apply).
• Valid BLS Certification (CPR).
• Associate’s or Bachelor’s degree in Nursing (BSN preferred).
• Experience in emergency, critical care, or admissions nursing is highly desirable.
• Strong clinical assessment and communication skills.
• Proficient with electronic health record systems and clinical documentation.
