Graduate Post Acute is seeking an Admissions Nurse for a full-time position in Philadelphia. The role emphasizes comprehensive initial assessments, clear communication with patients and families, and ensuring smooth transitions of care during admission.

Starting pay is $45 per hour with all-inclusive compensation. The position offers Monday–Friday 2pm–10pm shifts, tuition reimbursement, comprehensive benefits, and a supportive team dedicated to compassionate, patient-centered care.

Qualifications

• Active RN license (state-specific requirements)
• Valid BLS Certification (CPR)
• Associate’s or Bachelor’s degree in Nursing (BSN preferred)
• Experience in emergency, critical care, or admissions nursing is highly desirable
• Strong clinical assessment and communication skills
• Proficient with electronic health record systems and clinical documentation

Responsibilities

• Conduct comprehensive initial assessments of incoming patients to determine illness severity and care needs
• Collaborate with physicians and interdisciplinary teams to develop preliminary care and placement plans
• Serve as the first point of clinical contact, providing compassionate, patient-centered care during admission
• Communicate clearly with patients and families about the intake process, care expectations, and next steps
• Monitor for signs of medical distress during intake and elevate care as needed
• Maintain up-to-date knowledge of hospital admission policies, infection control protocols, and triage guidelines
