Graduate Post Acute is hiring an Admissions Nurse to join our team in a full-time role with a 2pm-10pm weekday schedule. You will conduct initial assessments, document medical histories, and collaborate with physicians to plan care and placement for patients.

This role emphasizes compassionate, patient-centered admission processes and clear communication with patients and families. The position requires an active RN license, current BLS, and a BSN (or equivalent) preferred, with experience in

• Active RN license (state-specific requirements apply).
• Valid BLS Certification (CPR).
• Associate’s or Bachelor’s degree in Nursing (BSN preferred).
• Experience in emergency, critical care, or admissions nursing is highly desirable.
• Strong clinical assessment and communication skills.
• Proficient with electronic health record systems and clinical documentation.

Responsibilities

• Conduct comprehensive initial assessments of incoming patients to determine illness severity and care needs.
• Collect and document detailed medical histories, including current medications, allergies, and previous treatments.
• Collaborate with physicians and interdisciplinary teams to develop preliminary care and placement plans.
• Serve as the first point of clinical contact, providing compassionate, patient-centered care during admission.
• Communicate clearly with patients and families about the intake process, care expectations, and next steps.
• Monitor for signs of medical distress during intake and elevate care as needed.
• Maintain up-to-date knowledge of hospital admission policies, infection control protocols, and triage guidelines.
